Use this form to escalate issues that cannot be resolved through direct engagement or present broader risks.

You should use this form when you are:

  • providing information to keep the Director informed of potential risks

  • making a recommendation, where a decision or approval is required

  • seeking approval to escalate the Issues Note to an Issues Brief or CEO Minute.

All Issues Notes should be concise, factual, include details of what has already been done to address the issue, recommended actions and any supporting evidence or correspondence.

  • clearly describes the issue and level of urgency

  • outlines what has already been done to manage or resolve the issue

  • includes any evidence, correspondence or supporting documents

  • identifies who has been informed or consulted

  • clearly states what action is required of the Director
